This Getting to Know You form allows you to:

• make a submission to Volcano’s Caregiver Program (where you can receive support for your caregiving responsibilities during your time with us);
• inform Volcano of your access needs and request accommodations to assist you to succeed in your work with us; and
• disclose any personal information that you would like your team’s leader and your team’s Stage Management team (if applicable) to be made aware of in advance of starting your engagement with Volcano
